2.1 Form Selection
Before Proceeding:
You must have a complete and accurate I-20 or DS-2019 to complete the Form I-901
online and to pay the SEVIS I-901 fee. If you do not have an I-20 or DS-2019 or if
the information on the form is incorrect, contact the school official or exchange
visitor program sponsor for a copy of the form before you continue.
You must be able to print the Form I-901 payment coupon or payment receipt when you
complete the form online. Ensure a printer is connected and working before continuing.
You cannot return to the page and print the coupon or payment receipt at a later time.

Caution:
Do not pay for a dependent child or spouse who is on an F-2, M-2, or J-2 visa. There
is no I-901 SEVIS fee due for a dependent child or spouse for these visa types.

Do not pay again if you know that you have made a mistake after you submitted your
information. Instead, send an email to fmjfee.sevis@dhs.gov and explain what
information may need to be changed.
Do not click the back button on your browser during your online session. Instead,
use the navigation buttons provided on each page.
Do not access other websites in the same browser window or open another browser window
during your online session. Your session will end.
Do not save or bookmark pages during your online session and try to access them later.
The saved webpage will not work.
Do not call or email the SEVIS Program Office to ask if your I-901 online payment
was received. Instead, click the "Check Status" button and you can check your status
online.

填写姓名，生日，DS2019 ID 号码，
填写完毕后，点击“Enter”进入另一页面。
2.3 填写邮寄及 DS 表详细信息
在这一页面中，共需要填写三方面的信息：1）邮寄地址信息，该地址要保证平邮信件可接受，如果
在后续的操作中，选择快递收取，则需要填写可接受快递的地址。注意快递需要额外支付 35 美金；2）个
人信息，包括性别，出生地，出生国，国籍；3）DS 表及护照信息，其中的护照号码可选填，所以在没有
护照的前提下，原则上也可以缴纳 SEVIS Fee，“Exchange Visitor Program”一项及“Exchange Visitor
Category”一项的所需信息均可在 DS 表中找到，所在位置请见 DS2019 表样表。
2.4 下一步是核对已填信息步骤，确认无误后点“Continue”
2.5 选择收据邮寄方式，如果是选择快递则填入联系电话，随后选择支
付方式，一般选择信用卡，使用信用卡支付时需要的信息包括：卡
号，卡片有效日期，及 3 位校验码（签名栏附近 7 数字的后三位）。
2.6 完毕后，打印付款确认页（即电子收据）
3. 查询支付状态，或电子收据打印
输入相关信息后进入：
点击“View”进入电子收据查看与打印
