Works at heights can be classified into four classes: 作业高度 heights 分级 classes 2～5米(含2米) between 2~5 meters (including 2 meters) 一级 Level 1 5米以上～15米 above 5~15 meters 二级 Level 2 l5米以上～30米 above 15~30 meters 三级Level 3 30米以上 above 30 meters 特级Special Level 3.2 作业单位：实施高处作业的施工单位及其上级管理单位或一级承包商。 The operation unit: The construction unit and its superior management unit or Class 1 contractor that carries out the works at heights. 3.3 属管团队：高处作业区域所在的业务团队或其它基层单位。 Responsible business team: The business teams or other units where the work at heights is performed. 4 职责 岗位职责下载项目部各岗位职责下载项目部各岗位职责下载建筑公司岗位职责下载社工督导职责.docx Roles and Responsibilities 4.1 HSE部提供高处作业安全管理标准并负责监督高处作业标准的落实情况；审批特级高处作业许可证。 HSE Department is responsible for developing the Height Work Safety Management Standard and supervising the implementation of the standard, and approving the work permits for the special level works at heights. 4.2 业务团队负责按照此标准确认高处作业安全措施，指定安全监护人；审批一、二、三级高处作业许可证。 The business teams are responsible for following the standard and ascertaining the safety measures for works at heights, designating the safety watch, and approving the permits for level 1, level 2 and level 3 works at heights. 4.3 作业单位负责实施高处作业，按要求落实高处作业安全措施，其一级承包商负责指定合格的安全监督人。 The operation unit is responsible for carrying out the works at heights, implementing the safety measures in accordance with the requirements. Its Class 1 contractor is responsible for designating eligible safety supervisors. 5 工作程序或要求 Work Procedures and Requirements 5.1 作业许可证 Work Permit 5.1.1 进行高处作业，必须办理《高处作业许可证》(见附表)。 When performing works at heights, “Permit for Working at Heights” (attached form) shall be applied for. 5.1.2 一、二、三级高处作业许可证由业务团队经理及其授权的人员负责审批。特级作业许可证由业务团队经理和HSE部安全主管及其授权的人员负责审批。 Level 1, Level 2 and Level 3 “Permit for Working at Heights” shall be approved by BT managers and his delegated personnel. Special Level “Permit for Working at Heights” shall be approved by BT managers and safety supervisor and his delegated personnel from HSE department. 5.1.3 《高处作业许可证 》最长有效期为7天，一个施工点一张作业许可证。 The effective duration of “Permit for Working at Heights” shall not exceed 7 days, and one work permit is only for one construction point. 5.1.4 《高处作业许可证 》一式三联，分别由属管团队安全协调员、安全监护人、作业单位现场负责人保存。属管团队联保存期为1年。作业期间，作业单位现场负责人应将其所持有的许可证用透明塑料文件袋封装悬挂于施工现场明显位置。 “Permit for Working at Heights” shall be made in triplicate, which are kept respectively by the safety coordinator of the responsible business team, safety watch and the responsible person of the site operation. The retention period for the copy of the responsible business team is one year. During the operation, the copy of the permit that kept by the responsible person of the site operation shall be closed in transparent plastic document bag and hung on site. 5.2 作业安全措施 Operative safety measures 5.2.1 凡参加三级、特级高处作业的人员，作业前应进行体检，无高处作业禁忌症。凡患有高血压、心脏病、贫血、癫痫等职业禁忌症者不得从事高处作业。 The personnel working at level 3 or special heights shall pass the health examination determining no height illness. The personnel with high blood pressure, heart disease, anaemia and falling sickness can not perform operation at height. 5.2.2 作业人员必须使用五点式安全带，戴好安全帽，衣着灵便。 The personnel working at heights shall wear quincunx safety belt, and safety helmet. The clothing shall be nimble. 5.2.3 安全带必须系挂在作业面上方的牢固构件上，不得系挂在有尖锐棱角的部位。当作业面上方没有牢固构件可以系挂安全带时，应在作业面上方拉设牢固可靠的“生命线”。 The safety belt shall be attached to the structure member above the operation place, and shall not be attached to the parts with sharp corners. If there is no structure member to hang the safety belt, should setup a firm and reliable “life rope” above the operation place to protect the operator. 5.2.4 安全带系挂点下方应有足够的净空。安全带应高挂低用，不得采用低于腰部水平的系挂方法。严禁使用绳子捆在腰部代替安全带进行高处作业。 There should be enough clearance space below the attachment point of the safety belt. Lowly hanged the safety belt can not be used as highly hanged, and shall not be attached below the level of waist. Rope wrapping around the waist can not be used in place of safety belt. 5.2.5 在邻近区域设有可能排放有毒、有害气体及粉尘超出允许浓度的排气口及设备的场合，严禁进行高处作业。必须作业时，应采取有效的防中毒措施。 For the places with vents and equipments that may discharge poisonous and hazardous gases and dust with concentration above the allowable limit in the surrounding areas, working at height is strictly prohibited. If the work has to be done, effective measures shall be taken to avoid poisoning. 5.2.6 高处作业要与架空电线保持规定的安全距离。 When working at heights, the prescribed safety distance shall be kept with the overhead wire. 电压等级 voltage 安全距离 safety distance ≤110 kv 2m 220 kv 3m 330 kv 4m 5.2.7 作业工具应放入工具套(袋)内，应有防止坠落的措施；较大工具应用绳子拴牢在构件上，不准随便乱放；在格栅式平台上工作应铺设垫板以防物件坠落。在同一坠落面上，一般不得进行上下交叉作业，如需进行交叉作业中间应有隔离措施（隔离材料应阻燃）。 Tools shall be put into the tool case (bag), and there are to be the measures for falling prevention. The bigger tools shall be toggled with structure member, prohibit leave them around. When working on bar flat roof, shall use isolation board to prevent from fallings. Cross operation up and down on the same falling plane is not allowed in general, there should be isolation measures in the between if the cross operation is required. The materials used to isolate should be not burn. 5.2.8 严禁上下抛掷工具、材料和杂物等，递送工具、材料和杂物等应用绳子系牢后吊送；所用材料要堆放平稳，必要时要设安全警戒区，并设专人监护。 Throwing tools, materials and sundries etc. high and low during the process of working at heights is strictly prohibited. Tools, materials and sundries etc. shall be tied to deliver. The materials used shall be stacked steadily, and safety caution area shall be setup with specially assigned personnel for supervision when necessary. 5.2.9 当高处作业位于人行通道或交通要道上方时，在作业面下方应设防护安全网以防坠物伤人。 When the height work is carried out above the passage, safety net shall be provided for falling things. 5.2.10 梯子不得缺挡，不得垫高使用。梯子横挡间距以30厘米为宜，下端应采取防滑措施。单面梯与地面夹角以60—70度为宜，禁止二人同时在梯上作业。如需接长使用，连接部位应按规范紧固。人字梯底角要拉牢。在通道处使用梯子，应有人监护或设置围栏。 Ladders shall not lack rungs and shall not be blocked up for using. A space of 30 centimeters between rungs is suitable for ladders, and anti-slipping measures shall be taken at the bottom of the ladders. An inclination of 60-70 degrees between the single ladder and the ground is suitable; It is prohibited for two persons to operate on the ladder at the same time. If extension is needed for the ladder, the connecting part shall be fastened according to the standard. The bottom of the trestle ladder shall be pulled firmly. When using ladders at the passage, there should be a person for supervision or rails be setup. 5.2.11 外用电梯、罐笼应有可靠的安全装置。非载人电梯、罐笼严禁乘人。高处作业人员应沿着通道、梯子上下，不得沿着绳索、立杆或栏杆攀登。 Reliable safety devices shall be equipped for the external elevator and cage. Carrying passengers in non-passenger elevator and cage is strictly prohibited. Personnel working at heights shall go up and down along the passage and stairway, climbing along rope, post or guardrail is not allowed. 5.2.12 高处作业人员不得站在不牢固的结构物(如石棉瓦、木板条等)上进行作业，不得坐在平台、人孔边缘和躺在通道或安全网内休息。楼板上的吊装孔、人孔应设坚固的盖板或围栏。在没有安全防护设施的条件下，严禁在屋架、桁架的上弦、支撑、檩条、挑架、挑梁、砌体、未固定的构件上行走或作业。30米以上的特级高处作业与地面联系应设置有专人负责的通讯设施或通信手段。 The personnel working at heights shall not operate by standing on the unstable structures (such as asbestos shingle and lath), shall not sit on the platform, the edge of manhole, or lie in the passage or safety net for rest. For special level works at heights over 30 meters, the communication facilities or means responsible by specially assigned personnel shall be set for the communication with the ground. 5.2.13 六级以上大风(含六级)和雷电、暴雨、大雾等恶劣气候条件下，禁止进行露天高处作业。 Under adverse weather conditions such as strong breeze with wind force higher than Force 6 (including Force 6) and thunderbolt, rainstorm and big fog etc. that may impact the safety of the construction, working at heights in the open air is strictly prohibited. 5.2.14 因事故或灾害需进行特殊高处作业，包括强风、高温、雨天、雾天、夜间、带电、悬空和抢救高处作业，要制定专门的作业 方案 气瓶 现场处置方案 .pdf气瓶 现场处置方案 .doc见习基地管理方案.doc关于群访事件的化解方案建筑工地扬尘治理专项方案下载 并经属管团队经理、机械设备部调度主管、HSE部安全主管批准。紧急情况下为抢救人员时，可由施工负责人或现场其他领导在确保救护人员安全的前提下口头批准，作业后立即报HSE部备案。 For special works at heights due to incidents or disasters, which including strong breeze, high temperature, rainy days, foggy days, night work, live-line work, suspending in the air and rescues, special operation scheme shall be developed and approved by responsible BT manager, Mechanical Department planning supervisor and HSE Department safety supervisor. Under emergency conditions, in order to rescue the personnel, it can be approved verbally by the responsible person of construction or other leaders at site that the safety of the rescue personnel is ensured. After the operation is completed, it shall be immediately reported to HSE Department for the record. 5.2.15 脚手架的搭设必须符合《建筑施工扣件式钢管脚手架安全技术规范》（JGJ 130-2001）。脚手架要牢固、稳定，保证在施工期间对所规定的荷载或气候条件下不变形、不摇晃、不倾斜，要有足够的面积满足堆料、运输、操作和行走的要求，确保作业期间的安全。主要要求如下： The erection of the scaffolds shall comply with JGJ 130-2001 “Technical Code for Safety of Steel Tubular Scaffold with Couplers in Construction”. The footing of the scaffolds shall be sound, rigid and capatable of carrying the maximum intended load. During poor weather conditions, the scaffold may in good condition. The scaffold shall provide sufficient place for pilling material, transportation, operation and walking to ensure safe operation. The requirements are following: 脚手架的搭设与拆除必须由取得架子工证的专门人员进行。 Any scaffolds must be erected under the supervision with the competent person. · 脚手架搭设完毕后应进行检查验收，经验收合格并挂标识牌后才允许使用。 After the scaffold has been erected, it can be put into service after accepted and attached information tag. · 脚手架材料：应使用钢质材料。钢管应采用符合《直缝电焊钢管》（GB/T13793-1992）或《低压流体输送用焊接钢管》（GB/T3091-2001）中规定的3号普通钢管，应无腐蚀、无弯曲、压扁或者裂缝。扣件应采用可煅铸铁制作的扣件，符合《钢管脚手架扣件》（GB15831-1995）的规定。脚手板材质应符合《碳素结构钢》（GB/T700-1998）中Q235-A级钢的规定。 Materials for construction of scaffold shall be made of steel which shall no corrosion, no bend, no stave and no crack to meet the requirement. Scaffold shall be erected in accordance with all industry and state regulations. · 脚手架的连接与固定：脚手架的立竿应垂直于垫板，垫板安置前需把地面夯实、整平。连接扣件螺栓要拧紧。使用起重装置吊重物时，禁止将起重装置和脚手架的结构相连接。作业层脚手板应铺满、铺稳（当脚手板长度小于2米时，可采用两根小横杆支承，但应将脚手板两端与横杆固定，严防倾翻）。 Erection: post shall be secured on level and a fixed pad vertically. Crane shall not be connected with scaffold structure when lifting. Toe board shall be required on work platform (toe board less than 2 meters long shall be secured on brace to avoid overturning. · 通行：脚手架应设置供作业人员上下使用的安全扶梯、爬梯或斜道。斜道两边、斜道拐弯处和脚手架工作面的外侧应设1.2m 高的栏杆，并在其下部加设18cm 高的挡脚板。 Walking and working surface: Safety claming equipments, guardrail, catwalk and ladder shall be provided for working personnel. Fall protection equipments such as Railing (1.2m high) and toe board (18cm high) shall be equipped for catwalk and working surface. · 临时照明：脚手架上禁止乱拉电线。必须装设临时照明时，金属脚手架应另设横担。 Temporary lighting: scaffold shall not be used for any job where exposed electrical parts may be present. If needed related equipments shall be erected for temporary power. · 拆除：脚手架拆除前，应在其周围设围栏，通向拆除区域的路段挂警告牌；高层脚手架拆除时应有专人负责监护；拆除工作应由上而下分层进行，拆下来的配件用绳索捆牢，并用起重设备或绳子吊下，不准随手抛掷。 Disassemble: warning fence and tags shall be provided for disassembled scaffold. Assign designated person to supervise.
