为确保我矿“双基”建设工作顺利开展～矿成立“双基”建设领导小组～矿长、书记任组长～生产副矿长、党委副书记、工会主席、纪委书记、掘进副矿长、机电副矿长、经营矿长、总工程师、总会计师、安监处长为副组长～由生产、经营各副总、党委工作部、人力资源部、财务部、企管部、行办、工会、团委、电视台、物业部、公安科、调度室、技术科、机电科、通防科、地测科、斜井项目部、安监处、环保节能办公室等单位负责人为成员～下设“双基”建设工作活动办公室～办公室设在安监处～安监处长兼任办公室主任～吕伦宝、屈学忠、刘坤房、魏常伟、亓增海、刘洪亮、肖培勤、吕宜新、张卫松、房公波、毕强为办公室成员～具体负责组织、协调、考核“双基”建设工作。 三、考核评分办法 1、安全生产“双基”建设实行百分制考核。每年至少有一项集团公司审查认可的安全管理创新或安全科技成果～每项加3分～没有项目扣3分,每半年有一个安全管理创新闪光点加2分～没有闪光点扣2分,开展质量 标准 excel标准偏差excel标准偏差函数exl标准差函数国标检验抽样标准表免费下载红头文件格式标准下载 3、平时上级安全监察机构、集团公司安监局、业务处室下矿检查情况作为基础资料纳入考核。被上级监察部门停止作业一次扣2分,被集团公司安监局或专业处室停止作业一次扣1.5分,被集团公司责任追究一次扣3分。 4、发生工亡一人次事故～当季否定～次季扣10分,发生一级非伤亡事故当季否定～次季扣5分,发生二级非伤亡事故当季否定。 三、奖惩办法 以集团公司检查结果为准: 1、季度被集团公司综合评定为前三名时～对领导小组组长各奖励1500元～对分管项目得分在95%以上时～对副组长各奖励1000元～对副总各奖励700元～对责任单位负责人、办公室成员各奖励400元～对责任人各奖励200元,低于95%时～分管领导、副总、单位负责人、责任人不予奖励。综合评定为后两名或取消评比资格时～由扣分专业的分管领导、副总、单位负责人、责任人承担集团公司罚款～由专业领导具体落实到人。 3、全年被集团公司综合评定为前三名时～对领导小组组长各奖励3000元～对分管项目得分在95%以上时～对副组长各奖励2000元～副总各奖励1400元～对单位负责人、办公室成员各奖励800元～对责任人各奖励400元,低于95%以上时～对分管领导、副总、单位负责人、责任人不予奖励。综合评定为后两名或取消评比资格时～由领导小组组长、副组长根据责任大小将罚款落实到有关人员。 3、全年未取得集团公司前三名但未被评为后两名时～矿对得分在95%以上有关人员奖励2万元～由领导小组按项目得分分配到人。 3 四、几点要求 1、各单位、科室必须高度重视“双基”建设工作～要结合“双基”建设标准及评分办法～制定出本单位“双基”建设规划措施～分工明确～落实到具体人员～规划措施一式二份～一份报“双基”建设办公室～一份备查。 2、各单位要根据分工积极开展工作～修订完善各项制度～做好各种记录～整理好纪要及考核结果～一项不落实罚款50元。分工负责项目未按时完成的一项罚款100元～并限期整改～否则加倍处罚。集团公司检查时～每扣1分罚款100元～并承担集团公司相应罚款。 3、各基层单位、科室部门每月进行一次自检～并将本单位负责项目完成情况、考核结果进行总结～于每月25日报安监处“双基”建设办公室～否则每拖一天对单位负责人罚款50元。自检不认真～一次罚款50元。 4、检查:现场检查由矿产经营公司、物业部、安监处组织每月对井上下安全质量、环境面貌和地面质量标准化检查验收一次～并形成纪要报“双基”建设办公室。资料检查由安监处组织“双基”建设领导小组成员单位有关人员及办公室成员每月至少检查一次～检查结果形成纪要备查。接通知要求～无故不参加检查的每次罚款50元。
