Banding\Bonding Appointment Check List
Please check off each step as completed . The purpose of this checkoff sheet is to make sure that we are as thorough as
possible thus reducing the number of potential emergency visits.
Ask parent if using Probanthine is permissible, if indicated
Read the chart to review for the procedure
Explain procedure to the patient
Remove separators and record the number removed
Pumice the teeth
Fit the appropriate bands (blank vs. attachments)
Band cementation - give 5 minutes to set
Clean all cement (occlusal, gingival, interproximal)
Cheek stretchers
Placement of braces
Light cure each bracket @ 45 seconds (2x interproximal and 1x occlusal)
Clean flash from around brackets
Place wires
Distal ends clipped
All molar band cleats checked
Proper home care instructions given **
Answer any additional questions that may be indicated
Chart completed with next appointment time
Speak with parent or patient
**If you can, the ideal time to begin oral hygiene instructions is when the bands and/or braces are curing. Please use the model with the braces for demonstration purposes. Try to have one of the parents present during the instructions.
- Brushing
- Flossing with the floss threader
- Wax placement if a distal end is poking or ulcer forms
- Identification of appliances
- Stress that each broken bracket can result in an additional treatment time
