湖州市初中毕业生综合素质 2006年湖州市初中毕业生综合素质 测评实施 方案 气瓶 现场处置方案 .pdf气瓶 现场处置方案 .doc见习基地管理方案.doc关于群访事件的化解方案建筑工地扬尘治理专项方案下载 (试行) 2006年，我市首批使用新课程的初中生面临毕业和升学。为了有效地衔接新课程改革，根据《教育部关于积极推进中小学 评价 LEC评价法下载LEC评价法下载评价量规免费下载学院评价表文档下载学院评价表文档下载 与考试制度的通知》和省教育厅有关积极推进中小学评价与考试制度改革的有关文件精神，在广泛听取意见的基础上，我市制定了《2005年湖州市初中毕业生综合素质考核实施意见(试行)》，市区各初中根据本《意见》要求，对2005年的初中毕业生试行综合素质考核，其中2%优秀学生作为市属一级重点普通高中的自主招生资格。通过试行，又认真学习国家教育部和省教育厅《浙江省国家基础教育课程改革实验区2005年初中毕业考试与高中招生制度改革的指导意见》(浙教办基〔2004〕101号)的精神，充分吸取首批课程改革实验区的实践经验，制定《湖州市2006年初中毕业生综合素质实施方案》，现印发给你们，并就有关要求通知如下: 一、综合素质评价的内容、原则、结果呈现方式、组织实施和结果运用 1、综合素质评价的内容:以道德品质、公民素养、学习态度与能力、交流与合作、运动与健康、审美与表现等方面的基础性发展目标为基本依据，根据学生的平时表现、活动记录、学习作品、特长潜能、奖惩情况等，力求全面、客观、公正地反映学生的总体发展水平。 2、综合素质评价的原则:一是坚持过程与结果相结合的原则，学校应当为每位学生建立《学生成长记录册》，全程、全面记录学生在初中阶段各方面的成长状况;二是坚持公开、公正、公平的原则，综合素质评价的内容、方法、程序等应在初三年级第一学期向学生及其家长公布，评价结果应告知学生本人及其家长，其中各类获得“A”等级的学生名单须在校内公示，如有异议，学校应及时进行调查、核实、处理。 3、综合素质评价的结果:评价结果由“综合评语”和“测评等第”两部分组成。 综合评语:主要对学生的道德品质、公民素养、情感态度、合作精神、日常表现等方面进行定性描述，尤其应突出学生的特点、特长和技能。各校可结合实际情况将其具体化，力求科学性与可行性。综合评语应当在学生自评、同学互评、家长评价并征求任课教师意见的基础上，结合《学生成长记录册》反映的情况，由班主任撰写，并对初中阶段表现写出是否合格的结论性意见，最后由学校领导审核确定。 测评等第:主要对能够体现学生素质发展水平的部分项目进行定量测评。测评项目包括艺术(音乐、美术等)、运动与健康(体育与健康等)、探究与实践(研究性学习、社会服务、地方课程、学校课程等)、劳动与技能(劳动技术、信息技术、实验操作等)四类。测评等第每类分A、P、E三等(分别代表优良、及格、不及格)，P、E等由学生所在学校确定，A等由学校在教育局指导下确定，E等比例控制在5,以内。 4、综合素质评价的组织实施:市教育局成立综合素质测评领导小组，市教育局分管局长任组长，成员由基教、教研、招办、教仪站、监察等人员组成。领导小组负责制定实施方案和相关政策，加强指导与监督，接受咨询、投诉与举报，及时处理综合素质评价中的问题。学校要制定实施细则并报市、县(区)教育局备案，确定专人负责，建立由学校领导、教师代表、学生代表和学生家长代表组to enterprises bigger and stronger. Focusing on improve the ability of an (4)在初中阶段参加过学校组织的比赛活动(美术方面)并获二等奖及以上奖次。(2005年9月至2006年4月间各校针对初三年级的比赛次数控制在2次以内。) (5)过程性测评达到5?者(5?人数不应超过班级总人数的20%)。 (二)体育与健康类 一、测评内容与方式 测评内容分为学习品质和运动与健康水平两个方面。 1、学习品质 学习品质的测评主要是由教师根据学生参与体育课和体育活动的态度和表现等，结合学生自评、学生互评，进行综合评定，测评结果分P、E两等，达到优秀、良好、及格都评定为P等，不合格为E等。评定标准如下: 优秀:认真上好体育课，课堂表现积极;积极参加早操、课间操和课外体育锻炼;学校规定的各项体育活动出勤率98%以上;有良好的自我锻炼习惯和方法，意志坚强。 良好:认真上好体育课，课堂表现较好;认真参加早操、课间操和参加课外体育锻炼;学校规定的各项体育活动出勤率95%以上;有自我锻炼习惯和方法。 合格:能上好体育课，课堂表现一般;能参加早操、课间操和课外体育锻炼;学校规定的各项体育活动出勤率90%以上，能坚持自我锻炼。 不合格:上体育课不认真或无故缺勤10%以上;其他体育活动不积极;学校规定的各项体育活动出勤率90%以下，缺乏自我锻炼的意志、习惯和方法。 2、运动与健康水平 根据《学生体质健康标准(试行方案)》要求进行测试，按学年完成。按照以下五类进行测评，测评总分在60分及以上的为P等，59分以下为E等。 第一类(必测):身高、体重 第二类(选测一项):台阶测试、1000米跑(男)、800米跑(女) 第三类(必测):肺活量 第四类(选测一项):50米跑、立定跳远 第五类(选测一项):坐位体前屈、握力体重指数、仰卧起坐(女) 二、测评结果的评定 1、学习品质、运动与健康水平测评结果均达到P等，则本类测评结果为P等;若学习品质、运动与健康水平测评结果中含有E等，则本类测评结果为E等。 2、学生在达到P等基础上并符合下列条件之一，其运动与健康类测评结果可评定为A等。 (1)在初中阶段经学校选拔参加过县(区)级及以上教育行政部门组织或认可的体育比赛或表演活动(大型团体操比赛或表演除外); (2)在初中阶段参加过学校组织的体育比赛(至少以年级为单位)并获得前三名(拔河、大型团体操比赛或表演除外); (3)认定的等级运动员; (4)《学生体质健康标准(试行方案)》测评总分在85分及以上。 3、学生确实因身体原因不能参加体育活动和锻炼，经县级以上医院证明，学校体育任课教师签名，学校主管领导批准，可免测，直接视作P等。 to enterprises bigger and stronger.
