1111 A lumbar laminectomy is an operation
which involves making an incision in the
middle of your back so that a lamina or part of
the vertebra can be removed. This opens up
the spinal canal and takes pressure off the
nerves in the back which may be causing pain
in your back and numbness in your legs.
2222 Wear and tear on the spine which is
sometimes age-related can cause the spinal
column to narrow. The joints on either side of
the spinal cord may swell and press on nerves
causing back pain and weakness in the legs.
The operation aims to improve mobility by
relieving the pressure on the nerves leading to
the legs.
3333 Will I have a scar? Yes, a very small scar
at the base of your spine. The scar is minimal.
You will have a small dressing over the suture
line on your back in case there is a small
amount of discharge after the operation. If the
discharge becomes discoloured and smelly,
you will have to take a course of antibiotics to
clear the infection. When you come back to
the ward after your operation you will have an
IV infusion or drip in your arm just until you
are able to drink fluids again. You will be able
to eat a soft diet as soon as the nursing staff is
satisfied that your bowels are working again
after the operation.
4444 Any surgery can cause some discomfort.
You will have Patient Controlled Analgesia
running through the drip in your arm for the
first day. When you are feeling uncomfortable,
you can press a small hand-held control button
and you will receive some pain relief through
the drip. After the first day you will change to
oral analgesia.
5555 We advise you to continue wearing your
anti-embolism stockings until you are up and
about again. Keep up the deep breathing
exercises too using your Tri-ball. You will
have a follow-up appointment between 6 and
12 weeks after the operation.
6666 The physiotherapist will assist you out of
bed the day after surgery. After that, nursing
staff will encourage mobilisation by helping
you to walk around the unit as much as
possible.

China’s Health Ministry issued a list of 307 essential drugs on Aug 18, 2009, as part of its
plan to create an essential medicine system in the coming 10 years to ensure people’s
access to essential drugs and cut their medical costs.
Essential medicines refer to those which satisfy the health care needs and are available to
the public at all times in adequate amounts and in appropriate dosage forms, at a price the
public can afford.
According to the plan, by the end of this year, about 30 percent of the state-owned
grass-roots health institutions would be equipped with all the essential medicines on
Tuesday’s list and would be asked to give priorities to using them when treating patients.
By 2020, all state-owned health institutions in both urban and rural areas should give
priorities to using essential drugs. “Such measures would ensure most of the rural citizens
with comparatively low incomes use cheap and safe medicines,” said Fu Wei, an official of
the Rural Health Department with the Health Ministry.
While the disposable income of China’s urban and rural residents grew close to 20 times
in the last two decades, their average medical costs soared to more than 130 times as
much as they used to be.
“The system would help safeguard people’s basic rights in using drugs, and promote the
integration of drug manufacturers and distributors,” said Chen Zhu, China’s Health
Minister. According to the ministry, the essential medicine list will be adjusted every three
years with respect to the changes of health demands. The drugs which have been
confirmed to cause serious adverse reactions would be removed from the list.
Building an essential drug system is part of China’s three-year plan on medical reform
unveiled last April, which aims to lay a solid foundation for equitable and universal access
to essential health care for everyone in China by 2020.

CardiopulmonaryCardiopulmonaryCardiopulmonaryCardiopulmonary RRRResuscitationesuscitationesuscitationesuscitation
Cardiopulmonary resuscitation (CPR) is an emergency procedure used to resume
breath and circulation by exerting artificial respiration and external cardiac massage. It is
widely spread and taught among health care workers and the public.
The procedure of CPR follows an ABC rule, that is, A for airway, B for breathing and C
for circulation. Assessment is the first step during the rescue and should be re-applied
before and after each step. At the beginning of CPR, rescuer needs at first to gently shake
the victim, shouting “Are you OK?” If the victim gives no response, emergency response
system should be called at once for further help.
Before starting CPR, the victim should be carefully moved onto the back. Then open
the airway by tilting the victim’s head back with one hand and lifting the chin with the other
hand. When head or neck injuries are suspected, choose the ‘jaw thrust’ way. By holding
the airway open, the rescuer needs to look at the victim’s chest, listen for sounds of
breathing and feel for air movement from the victim’s noses and mouth.
If no regular breathing is detected, twice mouth-to-mouth or mouth-to-nose breathing
rescue should be performed immediately. The artificial respiration should be delivered
slowly. And also check for signs of effectiveness such as chest rise and air escape during
exhalation.
Circulation assessment is followed after two breathing rescues. Index and middle
fingers should be put between the trachea and muscle at the side of the neck for carotid
pulse detection. When no normal pulse could be felt, chest compression needs to be
initiated right away. The heel of one hand should be placed on the lower half of the
sternum between the victim’s nipples and the other on top of the first so as to keep the two
hands parallel. 15 compressions should be performed continuously at a rate of 100 times
per minute.
A one-minute rotation of 15 chest compressions and two rescue breaths should be
performed before the second circulation assessment. If no sign of circulation is detected,
the rotation needs to be replicated. If the victim resumes normal pulses, no more chest
compressions are needed. Rescue breaths may be delivered if needed.
