002安全活动日记录大连远洋运输公司 DALIAN OCEAN SHIPPING CO. 安全活动日记录 SAFETY ACTIVITY-DAY RECORD 编号(No.):002 填写说明: 1、每月十五日定为公司安全活动日，由大管轮、大副主持召开部门的安全会议。会议之后，船长应主持召开船舶安全委员会会议。 2、会议内容如下(但不局限于这些): 1)总结当月的安全生产情况、事故及教训，提出具体的安全生产要求; 2)学习公司有关安全和防污染工作方面的文件、规章及港口国的规定，表扬重视安全工作的好人好事，批评违章、违纪的人和事; 3)会议期间，任何人都有权发表意见，主要针对工作中所发现的潜在危险，不遵守规章的做法，船体结构、机械设备及通讯、导航设备、救生消防设备等存在的缺陷，港口国检查发现的问题，内审、外审发现的不合格等进行充分讨论，分析原因，制定纠正措施; 4)对于船舶不能解决的问题，要采取临时安全措施，提出纠正方案，并立即报告公司主管部门。 3、会议的时间、内容、参加人员应予以记录记录。因值班未参加会议的人员，由部门长负责把内容传达给本人。每次部门安全会议结束后，由部门长把“安全活动日记录簿”交船长审签。 GUIDANCE TO RECORD ENTRY: 1、The safety meeting to be held by the C/O and 1/E at their respective department on the designed Company’s Safety thActivity Day, every 15 of each month, following the Onboard Safety Management Committee Meeting to be held by Master same day. 2、 The safety meeting should cover (but not limited to): 1)Summary of the safe operation condition in current month to be given, near miss, accident to be taken into discussion and analysis. Detailed safe operation requirement should be work out. 2)Study Regulations, Rules, company policy refer to the safety and anti-pollution, appraisal should be issued to the safety work condition, exemplar to be encouraged and violation to be criticized. 3)Anybody has the right to express his own idea during the meeting, mainly deal with the potential dangerous or actions that violate the rules and/or regulation. Any deficiencies of the structure, mechanism, navigation & communication equipment, fire fighting appliance, non-conformities during the PSC inspection, internal audit, external audit could be taken into for discussion and study, find the root cause and work out corrective actions. 4)For the deficiencies that could not be overcame by ship’s hands, the interim measure shall be taken and, report to the concerned department in the company without delay.. 3、Master, C/O and 1/E shall maintain the Record for safety activity-day, date/time of the meeting, topic and the attendee shall be recorded. C/O and 1/E is responsible for convey all information to the absent crew that was on duty during the meeting.
